Eye Surgery Center of New Albany collects any co-payments, un-met deductibles, and co-insurance amounts on the day of surgery. Please note that co-insurance amounts collected are an estimate; your total financial obligation will be determined by your insurance company and will appear on the explanation of benefits. If our estimated amount is less, we will bill you the difference. If our estimated amount is more, we will refund you the difference.

We encourage you to also contact your surgeon, anesthesia group and your insurance company if you have any questions about your financial obligations. In addition, please make sure your authorization/pre-certification for your surgery is on file prior to the surgery.

If you do not have insurance, you are responsible for the entire surgeon, facility and anesthesia fees. You will need to arrange payment with each entity separately. The surgery center cannot accept payment on behalf of another entity.

We will call you prior to surgery with the amount due on the day of surgery. Checks should be made payable to Eye Surgery Center of New Albany. We accept cash, check, Visa and Mastercard. We will leave a message on your home phone in order to make sure you have this information. If you have not heard from us prior to the surgery date, we will not be collecting co-pay on the day of the surgery. Late Arrival, Cancellation & No-Show Policy

The following guidelines help us provide timely and quality care to all patients.

Late Arrival

Patients are considered late if they have not arrived within 10 minutes of their scheduled appointment time.
Late arrivals may result in rescheduling or a reduced service depending on availability.

Cancellation

Appointments must be canceled or rescheduled at least 24 hours in advance.
Late cancellations may result in a $35 fee.

Missed Appointment

Missing an appointment without prior notice is considered a no-show.
No-shows may be charged a $35 fee.
If missed appointments persist this may affect future scheduling.
